A form used to schedule ongoing cleaning activity in order to maintain 5S in an area until a permanent solution can be found. The form lists the regular actions that need to be taken, the frequency of activity, person responsible, and clean activity needed. Using this form allows a company and its employees time to make changes that require lead time after a 5S workshop has taken place.

Enna’s forms in this 5S program are designed for practical use and to accomplish two main roles in a 5S program: involve and document. A pad / form consists of 25 - 11x14 sheets of paper with a cardboard backing.
